Services Provided.  Please review Our Services to learn about the services provided by Dr. J. John Hoy.  We perform medical, orthopedic and surgical services, as available and appropriate. Insurances Accepted.  Please check with your insurance to confirm that Jinwah John Hoy, DPM, NPI number 1104820653, is in-network with your insurance plan and whether a referral is required to be seen.  We accept most insurance plans, including Cigna, Aetna, Regence, Premera, Meritain, United Healthcare, First Choice, Kaiser Permanente, Lifewise, Providence, Ambetter and Medicare.  We only accept Medicaid as a secondary or tertiary insurance to another plan such as Medicare.  We do not accept Medicaid primary, or Medicaid managed care plans such as United Healthcare Community Care, Coordinated Care, Community Health Plan of Washington, Molina Healthcare, Wellpoint.

Washington State Labor and Industries patients: We only work with existing claims on a referral basis. We need a referral from your main treating doctor, which can be an urgent care clinic, primary care physician or chiropractor, as injuries may involve multiple body systems. Please go to an urgent care center if you don’t have a claim number to start a new claim; they will need to examine your whole body to see if there is any other injury. We do not start claims or accept L&I to start claims; we will only accept other insurance or private pay. Please have the Labor and Industries referral faxed to our office (206) 686-2184 from your treating physician, and provide your claim number. A secondary insurance or form of payment is also required for treatment in this office because L&I does not cover important indicated treatments for foot and ankle injuries.

Billing Procedures. Please check your insurance for copays, deductibles and coinsurances, as below.  We verify insurance for referral status, eligibility and patient responsibilities after you schedule your appointment and submit your paperwork.  Per industry standard, if we find that you have a known patient responsibility amount we will collect it at the time of service, then bill your insurance company for the remainder.  Payment is required at the time of service, so please be ready to remit each time you present for your appointment.

  1. Deductibles and coinsurances.  These are common with working age adults and some senior citizens.  We collect an estimated amount at the time of service for deductibles, coinsurances and certain services that are known to not be covered by insurances.  We then submit the claim to your insurance for processing, after which you may have a billable balance or account credit.
  2. Copays.  These are common with senior citizens and some working age adults.  These known amounts are collected at the time of service.
  3. Private pay.  If you are not insured or are otherwise paying privately, payment is due in full at the time of service.

Since fees are based on time and complexity, until the condition is evaluated, we are unable to provide complete information regarding total cost, or what is involved in a course of treatment.  There may be instances where the treatment of nails or calluses may not be covered by insurance.
